With COVID restrictions easing in some parts of the country, some of our clients have let us know that soon their stores will only be open to those who have received both doses of the COVID-19 vaccine.
To help determine which shoppers will be eligible to visit their stores, and to assist with assigning mystery shops in the next few months around these new government regulations, we kindly ask you to complete a short 2 questionnaire survey in regards to your current vaccination status, as well as your planned vaccination status come November 30th.
You are under no obligation to complete this survey if you do not wish, please note however that this may prevent being allocated available mystery shops as our clients may ask for proof of full vaccination to enter their sites/stores.

To complete this short survey, you can find the test on your shop log after logging in, please select ‘Tests and Qualifications’ near the top of the page as seen below:
Proceed to the Testing Center, select ‘Take Test’ besides Vaccination Status to proceed to the short survey.
